• Step 1: Fill out the form below and select the state where your company is registered.
• Step 2: Download and complete the tax certificate form for your state.
• Step 3: Upload/attach your completed tax form as part of this application.
• Step 4: Submit your completed application to our credit department.

NOTE: If your company is not based in the United States, the above steps for state tax certification does not apply to you. Just fill out the form, select your country, and submit to our credit department.
